How information about you helps us to provide better care - NHS England
Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.

This information along with your postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information. This allows those plannning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything.

We are trying to identify & support as many Carers as we can. If you’re a Carer who helps and supports someone your role is very important we want to ensure you get all the support you need. Please inform reception.

What is Carer?

A Carer is someone who, without payment provides help and support to a family member, partner or friend who could not manage otherwise because of frailty, long term illness, disability, mental health or substance misuse.

 If you are a carer you are entitled to an annual health check and influenza vaccination? Please contact our reception staff to arrange an appointment. 

We can also refer you to Sunderland Carers Centre for any support and advice.  Please enquire at reception.

Offer confidential advice, information and support service to carers of all ages, including young carers from the age of 5yrs, throughout the City of Sunderland. They have Support Groups to tackle social isolation, social activities and a variety of courses to help develop skills i.e. Confidence building, emotional resilience and caring for someone with Dementia.
