This allows those plannning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything.
If you have any concerns or wish to prevent this from happening, please speak to practice staff or ask at reception for a copy of the leaflet "How information about you helps us to provide better care"
More information can be found here www.nhs.uk/caredata
It is practice policy that all repeat prescriptions requests must be handed into the surgery (prescription box in the entrance) or posted to us using a stamped addressed envelope. We are currently taking prescription requests over the phone also.
If you have lost your repeat request counterfoil please write your name and address and the name of your medication on a piece of paper and hand this in the usual way.
Please allow 48 hours notice for all repeat prescriptions. You can collect and request prescriptions at any time during the normal surgery opening hours.
